Bulldog Athletics is currently transitioning to Teamworks for all required pre-participation paperwork. Please see the pre-participation physical form below. All physicals are required before any participation occurs. Physicals can be uploaded through your Teamworks account as part of the Medical History Form.

All other NCAA and Institutional forms will be sent to you through Teamworks.
