**MANDATORY ATHLETIC PARTICIPATION FORMS - 2025-26**

PLEASE READ AND COMPLETE ALL NECESSARY STEPS NO LATER THAN AUGUST 10, 2025:

To be eligible for competition during the 2025-26 seasons, please complete each section below by AUGUST 10, 2025. All sections are required to be cleared for athletics.  Thank you!!

1.  Click to complete the NCAA TITLE IX ATTESTATION - this is required annually by the NCAA for ALL student-athletes every year.

  • All incoming, continuing, and transfer student-athletes must complete an annual disclosure related to their conduct that resulted in discipline through a Title IX proceeding or in a criminal conviction for sexual, interpersonal, or other acts of violence. 
  • Transfer student-athletes also must disclose whether a Title IX proceeding was incomplete at the time of transfer.
  • Failure to make a full and accurate disclosure could result in penalties, including loss of eligibility to participate in athletics as determined by the member institution.
  • Please read carefully and choose the answers that pertain to your current and previous institution(s), including your high school.
  • You must be logged in to myRedlands in order to access the form, which means you need your Redlands login credentials.

2.  Click to complete your Bulldog Student-Athlete Online Biography Form - now includes a name pronunciation request.

  • This must be completed EVERY YEAR and is crucial to rosters, website content, and awards nominations.
  • You must be logged in to myRedlands in order to access the form, which means you need your Redlands login credentials.
  • Please email Director of Athletic Communications Lance Franey with questions regarding the biography form.

3.  Medical Clearance:  All student-athletes must complete the following to be medically cleared and to participate.  **PLEASE READ THE UPDATED INSTRUCTIONS!**

The medical history/PPE (physical) form is available here.  This form is a fillable PDF.  Please download and save to your computer when completing. This form can be uploaded to ATS. Please contact Todd Conger, Head Athletic Trainer, with any questions.  Please complete your history/PPE form BEFORE returning to campus.  

4. NEW TRANSFER STUDENTS (those entering the University of Redlands this fall from a 2-year or 4-year institution)

Please request FINAL, OFFICIAL transcripts from all previous institutions be sent directly to the UR Admissions Office AFTER spring grades have been posted. Hand-delivered or in-progress transcripts will NOT be accepted.  If you have NCAA eligibility questions, please email Associate Director of Athletics for Compliance Rachel Roche

5. INTERNATIONAL STUDENT-ATHLETES - if you have never enrolled at a Division III institution until now and meet the following criteria, please email Associate Director of Athletics for Compliance Rachel Roche

International student-athletes are required to complete an amateurism-only certification account through the NCAA Eligibility Center in order to be cleared for competition at a Division III school.  For this purpose, "international" is defined by the NCAA as meeting any of the following criteria: 

  • Attended high school outside the United States, its territoriesor Canada after age 11.
  • Your team was based and competed outside the United States (age 13 or older).
  • Your current address is outside the United States.

If you are an international student-athlete enrolling at the University of Redlands for the first time (first-years and transfers), you must create an account, complete the registration, and request certification before August 15, 2025.  Learn more about this process

6. FOR YOUR INFORMATION: NAME, IMAGE, and LIKENESS Policy.  The NIL Disclosure form is available to enrolled and rostered student-athletes only.
